What causes Pistoning of prosthesis?
Pistoning occurs when the residual limb slips up and down inside the prosthetic socket while walking. This can cause the skin to pull and be damaged. This is generally due to poor socket fit and inadequate suspension.
What is TSB socket?
Total surface bearing sockets Indicated for all types of stumps. TSB sockets “uniformly distribute weight over the entire residual limb” which therefore “delivers a minimal skin pressure”. TSB sockets are volume matched to the residual limb with 100% surface contact during the gait cycle.
What is a cuff suspension?
cuff A supracondylar cuff, affixed to a PTB socket, allows the prosthesis to hang on the patella. The suspension is indicated for people with vascular insufficiency, scarring, RL tenderness. Because it keeps the knee in flexion, it may not be appropriate for people with lax knee ligaments.
What are the pressure tolerant areas in a PTB socket?
The pressure tolerant areas include the anterior compartment, the medial tibial flare, and the patellar ligament (hence the name PTB). Areas that are not pressure tolerant include the fibular head, the hamstring tendons, and the tibial crest [4].
What is socket flexion?
Socket flexion/extension is named for the alignment it produces in the most distal residual joint. The prosthetist can align the socket in some “initial flexion” for an individual with short hip or knee flexors.
How much is a prosthetic socket?
Typically, the cost of the prosthetic socket ranges from $3,000 to $5,000 but can cost as much as $20,000 for more difficult fittings. The fitting process requires multiple trips to the prosthetist over two weeks to a couple months. In developing countries, a majority of amputees lack access to prosthetics.
How do prosthetic sockets work?
The prosthetic socket is the device that joins your residual limb (stump) to the prosthesis. The socket is made just for you, according to the condition and shape of the residual limb. For a prosthesis to work well it is important that the socket is a ‘good’ fit.
How do you cuff an amputee?
If they have only one hand, it might be handcuffed to their belt or a belt loop in their pants, if it’s sturdy enough to withstand that much stress. If this wasn’t an option because the cuffs couldn’t reach a suitable anchor, flex cuffs might be daisy chained together to get to such a point.
What are the pros and cons of prosthetics?
Joint prosthesis: typically used to replace damaged joints like: the hip, the knee, and the shoulder. Pro: takes the place of the worn out cartilage. and helps you still do the same work with out the pain. Con: the debris from the surgery can irritate the soft tissue around that area and cause inflammation.
What is abducted gait?
Abnormal wide base (abducted gait) (stance & swing) Stance between heels is abnormally far apart. Hip abductor contracture or spasticity. Proximal-medial device discomfort. Instability (patient has neuropathy and uses the wide base of support to avoid falling)
What is supracondylar suspension?
SC or supracondylar suspension keeps the prosthesis on the limb by holding on above the inside bone of the knee. The user wears a soft insert with a wedge that locks into the socket and keeps the prosthesis on the limb.
What is socket suspension?
This is when the suspension is created by the socket brim (top part) on the residuum. The brim part is made to extend and narrow over the joint (example over the knee joint) in order to create a grasp or hold over the joint. An example of this is the Patellar Tendon Bearing Supracondylar Socket. Only for transtibial suspension. Poor Suspension.
What is the supracondylar-suprapatellar (sc-sp) insert?
As described by these various authorities the supracondylar-suprapatellar (SC-SP) PTB includes a flexible insert and extends proximal to the patella and femoral condyles.
What is cuff suspension strap?
The cuff suspension strap ( Fig. 3 ) is, of course, an integral part of the PTB prosthesis as described by Radcliffe and Foort 6 and as such has come to figure prominently in the present day practice of prosthetics.
